POSITION SUMMARY
The Phone Triage Registered Nurse (RN) provides remote clinical assessment of patient symptoms, utilizing established protocols to determine the appropriate level of care, including home management, urgent care, or emergency services. The RN delivers patient education, facilitates prescription refills, and ensures accurate and timely documentation within the Electronic Health Record (EHR). Working under the supervision of the Clinical Director and/or Physician, the Phone Triage RN collaborates as part of a multidisciplinary team to support patient care. This role includes providing clinical support and technical guidance to medical and administrative staff.
ESSENTIAL FUNCTIONS

· Evaluate patient needs via telephone, using established protocols to provide safe, timely advice.
· Prioritize calls and advise patients on the appropriate level of care, including emergency guidance.
· Accurately record assessments, advice, and patient responses in the EHR.
· Communicate with physicians regarding patient concerns and schedule appointments.
· Offer self-care instructions for symptom management at home.
· Participate in continuing education and professional development to maintain competence.
· Actively conduct and/or collaborate in clinic research projects.
· Utilize current evidence-based practice guidelines to standardize care.
· Read, review, and participate in discussions of professional literature.
· Maintain ongoing collaborative efforts with other members of the healthcare team.
· Schedules office and hospital-based medical procedures and surgeries, for patients with appropriate provider and time/location slot, utilizing appropriate scheduling code when needed.
· Share unique knowledge and skills with other members of the healthcare team.
· Exercises clinical judgment in complex and uncertain situations, drawing on a broad knowledge base to identify and respond to potential life-threatening conditions.
· Use systematic and comprehensive assessment tools.
· Applies evidence-based protocols to guide decision-making.
· Other duties as assigned.
QUALIFICATIONS & REQUIREMENTS
· Current RN or LPN/LVN license in the state of practice.
· Usually requires 2+ years of clinical nursing experience, with previous triage experience preferred.
· Excellent communication, critical thinking, and the ability to work in a fast-paced environment.
· CPR certification is generally required.
· Strong interviewing, communication, and documentation skills are essential to effectively assess patients both directly and indirectly, ensuring safe, high-quality care through the nursing process.
